Liability waiver & release

I understand that Brazilian Jiu-Jitsu, grappling, and related martial arts training involve physical contact and carry an inherent risk of injury. I confirm that the participant is physically able to take part, and I voluntarily assume all risks associated with participation.

On behalf of myself, the participant, and our heirs, I release and hold harmless Island Jiu-Jitsu Oregon, Flow State MMA, and their instructors, staff, and property owners from any and all claims arising out of participation, except in cases of gross negligence. I also grant permission for photos or video taken in class to be used for promotional purposes.

If the participant is under 18, I confirm that I am their parent or legal guardian and I sign on their behalf.
